PROVIDER SAFETY
→ Please do not book an appointment if you are sick or have recently been sick.
Your safety and mine are always top of mind. From monthly health checks to vaccinations for COVID-19, HPV, and Hepatitis, I prioritize wellness so you can feel as secure as you do excited.
If you happen to have a scheduled appointment and get sick or start exhibiting potentially concerning symptoms prior to the appointment date, please let me know ASAP and you will get priority scheduling once you are feeling better.
Concerning symptoms include: sore throat, runny noses, coughs, excessive sneezing, eye infections, symptoms of any STD/STI - unusual discharge, discomfort or pain in the urethra or vaginal cavity, cold sores (or any sores around the mouth, nose, genitals), etc.
If you end up testing positive for Covid prior to an appointment, please note that you will be required to provide a recent cleared test. If you do not have access to a recent Covid test, you will be required to wait a minimum of 30 days from the original scheduled appointment to ensure the risk of transmission has ended.
If you come to your appointment with obvious symptoms of being sick you will be sent home and charged the full amount of the service booked.
By agreeing to these terms you are agreeing to not endanger me or my ability to work. By showing up sick you have automatically risked my ability to work for 7-14 days. Even if it was an accident, you will still be held liable. So it is in your best interest to reschedule at your earliest convenience if you do have any concerns, regardless how mild about risking my safety and job security.
CLIENT SAFETY
It is the clients responsibility to disclose any and all conditions, disabilities, medical anomalies that could potentially threaten their own well being during service. In the event that the client does not disclose any preexisting conditions, they forfeit their right to pursue any form of retribution, financial or otherwise, through the acceptance of these terms upon their submission of their screening application.
The client acknowledges and agrees to their responsibility to their own well-being during service and acknowledges that they will not agree to show up in an endangered state, and agrees that such actions would in turn endanger the provider.
The client understands and acknowledges that the provider of services does not own an Epi-Pen or any life saving medical equipment and therefore would not be able to act in a medical emergency of any state.
The client understands that by putting themselves in a potentially dangerous situation, they are equally putting the provider in a dangerous situation; upon that understanding they agree that they will not endanger the provider.
The client understands that the provider has the right to reject their application for any purpose, including but not limited to the question of the provider’s ability to provide a safe environment for both parties.
ENVIRONMENTAL SAFETY
The client agrees that they will not bring any objects, substances, or other foreign materials into any place of work that could threaten or endanger the safety of the provider, the client, or the client’s belongings (included, but not limited to: weapons of any form, drugs, toxins, chemicals, and animals).
